Stars are each setting's headline Care Compare rating (hospice: the CAHPS family caregiver survey rating; small hospices with too few surveys show none); hospital system ownership is PECOS-derived. Home health and hospice have no chain source in CMS data, so a blank chain there is correct, not missing.

Assisted Living

Every assisted-living and custodial-care building with an NPI — 35,000+ nationwide, each with a phone number, a named administrator, and chain rollups (NPPES registrations). ALFs are state-licensed, not Medicare-certified: no CCN, no star ratings, and no per-building Medicare utilization exists in any public dataset.

Buildings don't bill Medicare — the clinicians who round in them do. Two honest windows into that cohort, scoped to the state / territory zips above:
ⓘ Domiciliary E&M codes (99324–99337) were assisted-living-specific — Medicare deleted them after 2022, so that list is the last clean ALF-only cohort. From 2023 the same work bills under home-visit codes (99341–99350), which blend private homes and ALFs — Medicare doesn't say which building. Mobile rounding groups (NPs/PAs) dominate both lists: they're the incumbency map for facility BD, and a prospect list for wound care, DME, and point-of-care diagnostics.
