Check a provider or business against the Office of Inspector General's exclusion list: the List of Excluded Individuals/Entities (LEIE), the federal register of parties barred from Medicare, Medicaid, and every other federal health program. This search runs on the August 2026 file, straight from OIG's monthly refresh. Check by 10-digit NPI for a definitive answer, or search by name for potential matches.
They answer different questions, and this tool never blurs them.
Paste a 10-digit NPI and the answer is exact: on the current list or not on it. A clean result means not on the August 2026 file, never "never excluded". OIG replaces the file every month and reinstated parties drop off, so the date matters and we show it.
Most of OIG's file, about 9 in 10 rows, lists no NPI at all, so name search is the only way to see it. Every name result is labeled Potential match · verify with OIG because confirming identity takes OIG's SSN/EIN step, which we don't hold. Rows that do carry an NPI get a one-click definitive check.
• Results are checked against the LEIE file published August 2026; OIG refreshes it monthly.
• The LEIE is not SAM.gov: full federal screening also requires a
SAM.gov check, which this tool does not cover.
• Name matches are potential matches pending OIG identity verification.
• Not legal advice, and not a substitute for
OIG's official tool in a credentialing file.
A small number of excluded parties hold a §1128 waiver and may legally bill in the waived program or state; when a waiver is on OIG's file, this tool always shows it. Reinstated parties simply drop off the next monthly file, which is why every answer here is dated to the file it was checked against. The long version lives in our plain-English guide to the exclusion list.
Prospect 811 puts it next to what a rep or a compliance check actually needs: the provider's public billing identity, refreshed monthly, in one place.
