Where this data comes from. Provider identity, specialty, and practice location come from CMS's public NPPES registry (the National Plan & Provider Enumeration System), the official record of every NPI ever issued. Deactivated NPIs are excluded. Addresses are what the provider filed and can lag a move. Where a profile shows Medicare context, it comes from CMS's public utilization, Part D, and Open Payments files: traditional Medicare fee-for-service only, and CMS suppresses any provider-and-code line under 11 beneficiaries before publishing. An Open Payments record is a disclosed relationship, not evidence a clinician uses a product. Prospect 811 organizes public data; it doesn't add to or alter it.
