Where these numbers come from. Everything on this page is computed from CMS's public Medicare Physician & Other Practitioners — by Provider file for 2024, the most recent year released. CMS reports, for each provider, the share of their Medicare patient panel carrying each of 25 chronic-condition classes. "Est. patients" is panel size × that share, so a patient who sees three providers counts in three panels — these are panel-level estimates, not unique patient counts. Panel-prevalence estimate — these are providers whose patients carry this condition class, not providers who billed this exact code. Prevalence shares are top-coded at 75% and suppressed for small panels — a missing value never means zero. The data covers traditional Medicare fee-for-service only (no commercial insurance, no Medicare Advantage), and CMS suppresses low-volume lines before publishing. The code description is CMS's own ICD-10-CM wording. Comorbidity figures are ecological estimates from panel composition, not patient records.
